Exhibiting Artist Survey CAC & PMH Special Exhibition "*" indicates required fields I have read and agree to the following terms:Artwork cannot be removed from the exhibition prior to notification.* The prices and title of artwork listed on this contract form are accurate and will not be changed thereafter.* Commission rates are as follows: 60% Artist; 20% CAC; 20% PMH* Artworks are available for acquisition and works will be professionally framed, labeled and ready for installation.* Artists cannot sell the accepted artwork through their website for the duration of the exhibition. Artists will forward interested collectors to the CAC upon acceptance into the exhibition.* If artwork sells, Collector(s) will be responsible for shipping fees. Artists will be responsible for packaging the sold artwork, and dropping it off at their nearest fedex location.* The CAC will create and supply the artists with a pre-paid shipping label which they can affix to the package prior to drop-off.* Artist will promote their participation in this exhibition through various social media platforms, including but not limited to, Instagram, Facebook, and email campaigns. Optional CAC designed promo tools will be supplied at a later date.* I understand the CAC and Pasadena Museum of History may reproduce images of artwork to promote the exhibition.* Artwork will only be hand delivered on the designated drop-off days.* I understand that the Pasadena Museum of History has limited liability insurance.
